For women (mother, sister, wife) in case of different surname from mehrum then nikah or birth certificate is required as proof of relationship.

For minor (both male & female) birth certificate is required if the surname is different from mehrum.

For anyone who holds passport (any nationality) issued in the last 5 years MUST provide old passport (in case of lost passport police report will need to be submitted if not available then Statutory Declaration).
